1) Open and Print the Registration Form by Clicking Here. To save it to your computer, right-click/shortcut-click your mouse and select "save target as" (on Windows). The form is in PDF format so you'll need a PDF reader like Adobe Reader to open it. If you don't have it, you can get a FREE copy of Adobe Reader Here.
2) When filling out the form be sure to write or type Frank Gatto where it reads: Name of ISA__________________________. Without this name the form cannot be accepted. Sign form.
3) Fax your signed form back to us right away to 212-290-0253. This is a Secure and Dedicated fax line and is manned by The Learning Annex© Staff.

Once you fax your registration form, make a copy for yourself and mail the original to us at The Learning Annex©, 48 West 37th Street, New York, New York 10018 - Attention: Mr. John Goodfriend. Your faxed form will serve as a pre-registration until we get the original by mail. You must mail your original signed and completed registration form to us. Your registration will not be final until we receive it.
